Wuhan Novel Coronavirus - travellers who develop relevant symptoms, however mild, within 14 days of returning from mainland China, Thailand, Japan, Republic of Korea, Hong Kong, Taiwan, Singapore, Malaysia or Macau should self-isolate at home immediately and call NHS Direct Wales on 0845 46 47 or phone 111.  Travellers from Wuhan and Hubei province should self-isolate for 14 days, even if they do not have symptoms, due to the increased risk from that area.

Order Prescriptions online

My Health Online is an online service brought to you from NHS Wales.  At this practice, it offers patients the convenience to 

  • Order repeat prescriptions,
  • Book routine GP appointments at War Memorial Health Centre,
  • Print your immunisations record, your current medications and a brief medical record 
  • Update personal details including changes to address, email address and mobile telephone numbers.

If you wish to register to use the My Health Online Prescription Service, please come into to the Surgery to request a My Health Online Account Form. Alternatively, you can print a copy below and bring it into the surgery.  Please bring proof of identity documents with you.

Proof of Identity is necessary to protect patient confidentiality. Identification documents should confirm your full name, residential address and date of birth. Ideally, this will include photo ID such as a passport or new style driving licence.
