VACCINATIONS FOR TRAVELLERS
Patients are requested to read this information and complete a 'Travel Risk Assessment form for vaccines for travel'. Once we have received your completed form we will contact you within 2 weeks, with an appointment date for a travel vaccine, if needed.
Crickhowell Group Practice is able to provide a basic provision for patients wishing to travel abroad and can provide all of the NHS vaccines.
Please note, this practice is not a travel clinic therefore, if specialist travel advice is required or specialist vaccinations are needed (for example rabies, yellow fever) this must be sourced from a specialist travel clinic. Malaria prevention tablets can be purchased from most high street chemists.

Please allow at least 6-8 weeks prior to your departure date to help us try to meet your travel vaccination needs.
If you have not already done so, please check that your contact details are up to date and that you have given us permission to contact you by Text message and Email.
The 'Travel Risk Assessment form for travel vaccines' can be downloaded here or collected from Reception at War Memorial Heath Centre.
Please complete the form and return to the Reception desk at the War Memorial Health Centre, or you can scan the completed form and email it to Info.Crickhowell@wales.nhs.uk
