Registration With The Practice

PreRegistration


When registering with the practice, we ask that you bring your medical card or documents showing your NHS number and place of birth along with details of your previous general practice (GP).  You will also be asked to provide additional proof of identification.

We request that new patients complete a health questionnaire that will provide us with important details about your health pending receipt of full records from your previous GP. We do this as it can take a considerable time for us to receive your medical records.  If you are currently on repeat medication we will also need to see your last prescription slip.

Once registered, you will be offered an appointment to discuss your health with your new practitioner. There may be several forms to complete during this registration process - please ensure that the details are accurate.

To save time, you can print the Registration Pack below, complete, sign and return it to the surgery in person when you register.

Alternatively, you may ask for a Registration Pack at Reception and we will be happy to provide one for you to complete.
